RULE 2.0
FILING OF PLEADING, MOTIONS AND DOCUMENTS
A. In the initial pleading filed by a party or counsel, the party or
counsel filing the pleading shall include the party’s or counsel's signature,
the name of the party or counsel, the party represented, and the party’s or
counsel’s address, telephone number, fax number, e-mail address and
attorney registration number.
B. The clerk shall not accept any pleading that does not comply with the Civil Rules, Local Rules or any orders of the Court. B. Each complaint, cross-complaint and counter-claim shall state in the caption of the case the nature of the action from the list of type of cases in listed in the Appendix A to this rule. The case designations are for the internal use by the Court and Clerk of Courts and do not affect the merits of any pleading. Information relative to a case designation shall appear in numerical order immediately following the pleading designation.C. When a case is a re-filing of a previously dismissed case the caption of the case shall indicate that this is a re-filing, indicate the previous case number and the judge previously assigned. D. Each complaint, cross-complaint and counterclaim where a jury has been demanded, shall so designate immediately below the case designation with the following language: "Jury Demanded," or in accordance with the Ohio Civil Rule 38, which shall not be deemed to replace the jury demand itself. F. The Court and clerk do not accept filing by facsimile transmission. G. A sample caption appears in Appendix B to this rule. H. Counsel of record shall keep the Court informed, in a separate notice, of current address and phone number.
